Mitsui Chemicals extends heartfelt sympathy to the family of the deceased and sincerest apologies to the injured and those suffering damages to homes and property, local community residents, persons from nearby companies, the authorities, our customers, our shareholders, and all stakeholders affected by the tragic explosion and fire at the Mitsui Chemicals Iwakuni-Ohtake Works on April 22 (2:15am).
On April 24, Mitsui Chemicals established an Accident Investigation Committee, comprised solely of members of the academia and external specialists, to investigate the accident to determine causes and to develop measures to prevent future recurrences.
To date, the Committee has held six meetings and has determined causes of the accident and approved measures developed by Mitsui Chemicals in response to the causes of the accident to prevent future recurrences.
Following the conclusion of the Committee investigation, Mitsui Chemicals completed an official report on cause of the accident and proposal of preventive measures in its [Report on the Investigation of the Explosion and Fire Serious Incident at Resorcinol Production Facility] and submitted it to the proper authorities on August 16 for review. The report was officially approved by the proper authorities on August 20.

The Accident Investigation Committee will continue its investigation of other factors leading up to the accident and expects to compile its findings in a report on the Iwakuni-Ohtake Works accident.
